A prominent Yuma, Arizona physician, Dr. Irfan Fazil, and his wife, Nadia Hanif, are at the center of a sweeping healthcare fraud investigation that alleges they defrauded taxpayers of millions of dollars through a network of clinics.

Fazil, who was twice recognized as a top kidney doctor, was arrested on June 3 and faces felony charges including fraud, theft, conspiracy, and drug offenses. He is currently held on a $50 million bond. Hanif, who co-managed their clinics, is believed to be in Pakistan and has not been formally charged.

Prosecutors describe the couple as the masterminds of a large-scale criminal enterprise that allegedly generated tens of millions of dollars through fraudulent practices. Between July and December 2025, Fazil's clinics reportedly submitted over $36 million in claims to Arizona's Medicaid program, accounting for approximately 97% of the business's revenue during that period. The alleged scheme involved billing for services such as X-rays, ultrasounds, and ECGs that patients claim were never performed. Some patients stated they were out of the country when the billed services supposedly occurred.

Investigators are examining Fazil's international connections, including a clinic owned by Hanif in Dubai and alleged money transfers of approximately $1 million to Dubai and Pakistan. Fazil also reportedly utilized a private fleet of aircraft for travel and has alleged ties to Mexican drug cartels through his pilot and associates, raising concerns about his potential flight risk.

Authorities have seized Fazil's private aircraft and luxury vehicles and frozen his bank accounts. The alleged fraud has drawn comparisons to the 'Feeding Our Future' scandal in Minnesota, where significant funds intended for child nutrition programs were allegedly stolen and funneled overseas.

Fazil cultivated a public image as a respected doctor, appearing on his own YouTube health show and co-publishing a health magazine with Hanif. The couple resided in a $600,000 villa in Yuma. Despite Fazil's arrest, the Bio Family Clinic remained operational, causing distress for patients seeking ongoing medical care and records.

Former employees have reported losing paychecks and access to retirement savings. Some staff indicated that the alleged fraud, marked by a sharp increase in claims and a traceable paper trail, might have been evident to those within the organization.
