A young student's decision to treat what he believed to be the common cold led to a life-threatening paracetamol overdose, ultimately requiring him to be placed on life support and learn to walk and talk again. The incident underscores the potential dangers of exceeding recommended dosages of over-the-counter pain relievers, particularly when combined with other medications containing the same active ingredient.

Riley Lubas, 20, a student at the University of Bath, purchased both cold and flu tablets and standard paracetamol when he began feeling unwell. Unbeknownst to him, the cold and flu medication also contained 250mg of paracetamol per dose. Over several days, he took two tablets of each medication every four hours, significantly exceeding the maximum recommended daily intake of 4000mg of paracetamol. His intake reached approximately 9000mg daily.

Initially experiencing stomach pain and an inability to keep food down, Lubas's flatmates urged him to seek medical attention. At the Royal United Hospital, tests revealed liver abnormalities, and he was admitted. His condition rapidly deteriorated, leading to his transfer to the intensive care unit. His parents, Chris and Kate, were informed that his liver was heading towards failure, a critical state that can be fatal within days.

Doctors later transferred Lubas to King's College Hospital in London, where his condition became critical. He was sedated, placed on life support, and undergoing dialysis due to multiple organ failure. With up to 80% of patients with multiple organ failure not surviving, his family feared the worst. His parents recalled being asked about his medication intake, which led to the realization of the accidental overdose.

Lubas was placed at the top of the UK's liver transplant list. However, due to heart damage, he could not undergo the procedure for two days, during which he remained under close monitoring. Doctors described the liver damage as nearly irreversible, but against the odds, his liver began to improve. His youth and underlying good health were credited for his recovery, allowing him to come off dialysis and life support after a week.

The recovery process was arduous. Lubas underwent a tracheostomy and had to relearn basic functions like walking and speaking due to the toll organ failure had taken. After weeks of intensive care and rehabilitation, he was discharged. A year and a half later, he has made a full recovery and returned to normal life. His parents have since completed a 50km hike to raise funds for the King's College ICU.

Paracetamol overdose is a significant public health issue in the UK, accounting for approximately 100,000 emergency room visits annually, with an estimated 150 to 250 deaths each year. Studies indicate that around a quarter of adults have exceeded the recommended daily dose. The toxic byproduct of paracetamol metabolism, NAPQI, can overwhelm the liver in excessive amounts, leading to catastrophic organ damage. Symptoms of overdose can initially mimic cold symptoms, including sickness and vomiting, but can escalate to liver failure and multiple organ shutdown. Lubas's family is now advocating for greater awareness of the risks associated with paracetamol, especially among young people, highlighting how easily it can be to unintentionally exceed safe limits when taking multiple medications.
