Stopping GLP-1 medications, such as Ozempic and Wegovy, abruptly can lead to immediate metabolic changes and a potential return of weight gain, according to medical professionals. These drugs, widely sought for significant weight loss, can cause an increase in appetite and a rise in blood sugar levels for diabetic patients as they leave the system.

Dr. Jumana Al-Deek, a board-certified family physician, explained that patients coming off these medications may experience an increased hunger compared to before. For individuals with obesity, this can also result in the recurrence of weight-related conditions that had previously resolved. A particular concern highlighted by Al-Deek is the potential loss of lean body mass, which plays a crucial role in maintaining satiety and supporting metabolism. Without adequate resistance training during weight loss, individuals may find they have less muscle mass to help manage appetite and metabolic function after discontinuing the medication.

Some patients who stop GLP-1s and later wish to restart may find that the medication is less effective, potentially requiring higher doses to achieve their weight goals. While there is no definitive evidence of tolerance developing, regaining weight can make it more challenging to return to a target weight.

To mitigate these risks, doctors emphasize the importance of a structured maintenance plan developed before discontinuing the medication. This plan should prioritize protein intake and incorporate resistance training at least twice a week to preserve lean body mass. Al-Deek also advises patients to focus on getting sufficient sleep, continue structured meal planning to avoid reverting to old habits, and monitor their weight regularly to detect any regain.

The strategy, according to health experts, should shift from solely focusing on weight loss to emphasizing weight maintenance. By focusing on resistance training, adequate protein consumption, and healthy eating habits, it is possible to prevent significant weight regain after stopping GLP-1 medications. The widespread use of these drugs has been noted, with one KFF poll indicating that one in eight adults are currently taking them, and many celebrities have publicly acknowledged their use.
