Women in their thirties should be advised that even a single glass of wine per night could elevate their risk of breast cancer, especially if combined with weight gain, according to new research led by Imperial College London.

The study, published in the journal Scientific Reports, analyzed data from over 110,000 women and found that while alcohol consumption and smoking often peak in late adolescence, body fat steadily increases throughout adulthood. This accumulation of body fat, particularly in mid-life, is a significant concern as it is a period when cancer-causing fat is likely to accumulate.

Around a quarter of breast cancer cases are believed to be preventable through lifestyle modifications, including reducing alcohol intake and maintaining a healthy weight. However, this research highlights that the impact of certain risk factors is more pronounced at specific life stages.

While women in their twenties may consume more alcohol than any other age group, researchers identified that women in their thirties face a heightened risk due to the combined effect of alcohol and excess weight. This finding comes amid a nearly 10% increase in breast cancer cases among women aged 20 to 49 over the past decade.

Professor Marc Gunter, the study's lead author, emphasized the need for public health messaging to be tailored to moments in life when women's habits typically change. He noted that adolescence and early adulthood are critical for encouraging physical activity and preventing rapid weight gain. For women in their thirties to fifties, focusing on managing body weight and curbing alcohol consumption becomes paramount.

Obesity is a known risk factor for at least 13 types of cancer, including breast cancer. Fat cells produce aromatase, an enzyme that converts testosterone into estrogen. Elevated estrogen levels can promote cell division and multiplication, thereby increasing cancer risk. Research indicates that more fat cells lead to higher estrogen levels and a greater risk of breast cancer, an effect particularly strong after menopause. Obesity in post-menopausal women is associated with a 30% higher risk of breast cancer compared to non-obese women.

Alcohol's contribution to cancer risk is multifaceted. When metabolized, alcohol produces acetaldehyde, a toxic chemical that can damage DNA and lead to mutations. Alcohol also raises estrogen levels, potentially fueling tumor growth, and can enhance the harmful effects of other carcinogens, such as those found in tobacco.

Even one glass of wine a night, equivalent to one to two units of alcohol, has been shown to increase breast cancer risk by 15%. This risk further escalates by approximately 10% for each additional daily drink.

Professor Amy Berrington from The Institute of Cancer Research suggested that warning women about age-specific alcohol-related risks could have broader public health benefits, potentially preventing other diseases like diabetes and cardiovascular disease. "Public health strategies would have a much broader impact on women’s health," she stated.

In the UK, approximately 59,000 new cases of breast cancer occur in women annually, with about 420 in men. Key signs include a lump or swelling in the breast, upper chest, or armpit, changes in breast skin, alterations in size and shape, or nipple discharge. While there isn't a single cause, known risk factors include age, family history, alcohol consumption, smoking, obesity, certain hormonal treatments like the contraceptive pill or HRT, and breast density.

Women in the UK are invited for NHS screening every three years after their 50th birthday.
