Charlie Sheen writes in his new memoir that he underwent liposuction after an escort called him “fatso” during a 2000 trip to Toronto to film the biopic Rated X.

In The Book of Sheen, the actor recounts that on his final night in Toronto he ordered an escort through what he described as “the matchmaking Yellow Pages of Bell Canada.” After a sexual encounter, Sheen said the woman slapped his stomach and asked, “Whattya say, Fatso — you up for round two?” He wrote that the remark devastated him and prompted a visit to a liposuction surgeon about a week after he returned to Los Angeles.

Sheen recalled the encounter while describing the filming of Rated X, in which he and his brother Emilio Estevez portrayed the Mitchell brothers, figures from the adult film industry. He said he had gained weight at the time because he was newly sober and that weight gain is a common occurrence for people recovering from stimulant use. "I'd rather be a bit soft and alive than crack-skinny and f—ing dead," he wrote.

In the memoir, Sheen details his struggles with cocaine and alcohol addiction and describes a colorful personal life. He said the escort’s offhand comment about his appearance left him reeling. "Wow. Jeez, lady, you had 10,000 nicknames to choose from and you went with that?" he wrote. "I wanted to blow my brains out. It’s amazing where one word can send a guy." He added that the remark did not stop him from continuing the encounter.

Sheen has publicly discussed periods of substance abuse and rehabilitation in the past, and The Book of Sheen adds personal anecdotes and reflections on those years. The actor, best known for roles on television and in film, frames the Toronto incident as an example of the small moments that influenced his decisions during recovery.
