The Welsh Gender Service has halted scheduled and new referrals for gender-affirming surgeries due to concerns over referral numbers. Cardiff and Vale University Health Board, which oversees the service, reported that data from 2025-26 indicated "significantly higher rates of referral for gender affirming surgery... compared with English gender services."

The NHS Wales Joint Commissioning Committee has requested an independent clinical review after identifying "matters" through routine "assurance processes," according to the health board. Concerns were also raised regarding aspects of the referral and assessment processes.

As a result, new referrals for gender-affirming surgery, as well as surgical assessments and previously scheduled surgeries, have been paused. The Welsh Gender Service, established in 2019, provides specialist care for adults in Wales and refers patients to England for surgical procedures.

The health board acknowledged that the announcement may cause concern for patients, families, staff, and the wider community. However, it stated that other components of the service remain operational, including hormone therapy support, psychological support, and speech and language therapy. Patients with existing appointments are encouraged to attend for discussions regarding next steps and psychological support.

Cardiff and Vale University Health Board has been asked to immediately pause referrals from the Welsh Gender Service for gender-affirming surgery and existing scheduled surgeries while the independent clinical review is conducted. The review aims to provide assurance regarding assessment, referral, and governance arrangements.
