Taylor Swift unveiled her latest music video, "Patient Zero," at the MTV Video Music Awards 2026 on Sunday. The video features Dakota Johnson and Colin Farrell and was previewed in a short clip on "CBS Mornings" earlier in the week.
In the narrative short, Swift portrays the ghost of Farrell's character's deceased wife, attempting to warn Johnson's character about her husband. The video opens with Swift and Johnson, dressed in matching gray outfits, at a graveyard. As the story progresses, Swift's character becomes visible only to Johnson, shadowing her and offering comfort as she experiences symptoms of poisoning.
One scene depicts Farrell's character interacting flirtatiously with a woman played by Cara Delevingne, followed by a confrontation between Farrell and Johnson, observed by Swift's spectral character. Later, at a gala, Johnson's character sees Swift's ghost, leading to her collapse and subsequent public embarrassment. The ensuing car ride home sees Farrell accusing Johnson of being drunk and deliberately humiliating him.
A flashback reveals that Farrell's character poisoned and killed Swift's character, explaining the significance of the grave visited earlier in the video. This revelation parallels the current marital strife depicted.
Johnson, who was recently reported to have used Swift's New York apartment to meet with Machine Gun Kelly, described working on the video as a "fun thing on the planet" due to her established friendship and creative respect with Swift.
Swift directed the "Patient Zero" video, collaborating with Oscar-winning cinematographer Emmanuel Lubezki. The choice to debut the video at the VMAs coincided with Swift receiving the inaugural MTV VMA Artist Director Honors, an award celebrating artists who have "expanded music videos and advanced visual storytelling" through their directorial work.
Swift is now the most-awarded solo artist in VMA history. Johnson presented Swift with the new honor before they watched the music video premiere together.
Hints about the project had been circulating for months, appearing in Swift's "Opalite" music video and social media posts. She also included subtle clues during a surprise cameo on "Law & Order: Special Victims Unit" at the 2026 Emmys.
Swift officially announced the single "Patient Zero" on social media on Tuesday, followed by the announcement of "The Life of a Showgirl: The Encore," an expanded version of her 2025 album featuring four new tracks: "Patient Zero," "Cleveland!," "Pink Clouding," and "Babylon." The expanded album was released on Friday.
