Fans have drawn parallels between "Patient Zero" and Swift's 2022 song "You're Losing Me," which is widely believed to be about Alwyn. Both songs feature medical terminology, with "You're Losing Me" containing the lyric, "I can’t find a pulse, my heart won’t start anymore," while "Patient Zero" includes, "When the toxins take their hold / You’d rather die than let it go." Another fan observation noted similarities between "Patient Zero" and Swift's 2019 track "Cruel Summer," where she referred to Alwyn as "grinnin’ like a devil."
