Reality television personality Porsha Williams has revealed she underwent an emergency partial hysterectomy after doctors discovered 21 fibroids in her uterus. The 45-year-old star of The Real Housewives of Atlanta detailed the procedure in an essay for Glamour Magazine, stating that the removal of her uterus, while life-saving, also means she will grieve the loss of her ability to have more children.

Williams described how the fibroids, including one as large as a cantaloupe, were found when she accompanied a friend to a fertility appointment. During an examination, doctors identified over 15 large fibroids, along with endometriosis and a uterine lining condition. She was subsequently scheduled for an MRI and later informed that a hysterectomy was necessary.

This is not the first time Williams has dealt with fibroids. She was previously diagnosed before her first pregnancy with her daughter Pilar, then seven. At that time, a fibroid had outgrown her pregnancy, leading to a miscarriage at four months. After the birth of Pilar, whom she shares with ex-fiancé Dennis McKinley, the fibroids returned. Williams underwent embolization, a procedure to block blood supply to shrink the tumors.

Following the partial hysterectomy, which allowed her to retain her ovaries, Williams reported a significant improvement in her quality of life. She noted that the fibroids had been contributing to migraines, low iron levels, and significant physical stress. The condition had also impacted her intimate life, with the fibroids growing large enough to obstruct over 60% of her birth canal and put pressure on her bladder.

Williams expressed her sadness over not being able to have more children, having always envisioned a large family. She had hoped for one more child after Pilar, feeling optimistic about her ability to do so given her age, support system, and financial stability.

In other recent developments, Williams finalized a divorce settlement with Simon Guobadia last month. She secured a seven-figure payment and sole ownership of their Atlanta mansion. Court documents indicate she received $1,568,181.63 as reimbursement for Guobadia's share of the home's equity and took title to a 2021 Rolls-Royce Ghost.

Williams and Guobadia had separated in February 2024 after 15 months of marriage. She also recently went public with her new girlfriend, Patrice 'Sway' McKinney, in November 2025. The two initially connected via Instagram in 2018 and rekindled their relationship years later.

A hysterectomy is a surgical procedure to remove a woman's uterus. There are different types, including partial hysterectomy (removal of part of the uterus), total hysterectomy (removal of the uterus and cervix), and radical hysterectomy (removal of the uterus, cervix, and vagina). The procedure is most common in women aged 40-49 and is performed for conditions such as fibroids, endometriosis, uterine cancer, chronic pain, or bleeding. In some cases, ovaries and fallopian tubes may also be removed. If ovaries are removed, it can induce menopause regardless of age, potentially requiring hormone therapy.
