NHS Staff May Have Breached Medical Records of Tiger Attack Victim
Yorkshire Ambulance Trust investigating potential unauthorized access to patient information following a fatal incident.
The Yorkshire Ambulance Trust is investigating two potential incidents where staff may have accessed or attempted to access the medical records of William Chapman, a 24-year-old man who died after a tiger attack at Yorkshire Wildlife Park. Chapman was fatally injured on Saturday at the park near Doncaster.
The trust became aware of the potential breaches following Chapman's death and has launched an investigation to determine the full circumstances. A spokesperson for the trust stated that they believe two staff members may have accessed or attempted to access incident/medical information without a legitimate reason.
Chapman died from "multiple traumatic injuries and animal interaction" after entering the tiger's enclosure, an inquest into his death heard on Thursday. The trust has informed Chapman's family of the potential breaches and offered condolences.
Yorkshire Ambulance Trust emphasized its commitment to patient confidentiality and information security, noting that robust systems are in place and that these potential issues were identified through proactive audits. The Information Commissioner's Office has been notified, and the trust will continue to update Chapman's family as the investigation progresses.
