New details have emerged regarding the death of Blake Garrett, the actor best known for his role as Plug in the 2006 film "How to Eat Fried Worms." Garrett, 33, died in February from an accidental fentanyl overdose, according to an autopsy report. The report indicates that drug paraphernalia was found near Garrett when he was discovered unresponsive in his sober living home in Tulsa, Oklahoma.

According to the report, Garrett was found near his bed with rolled dollar bills, a lighter, and foil exhibiting burnt residue. Alcohol testing at the time of his death came back negative. The Oklahoma Office of the Chief Medical Examiner's report also noted swelling in Garrett's brain and lesions in various stages of healing from shingles.

Garrett's mother had previously expressed concerns to TMZ that her son might have been self-medicating. She stated that Garrett had been hospitalized the week before his death and diagnosed with shingles, a painful rash. At the time of his death, Garrett had reportedly been sober for three years.

Garrett, who passed away in February, was best known for his role in "How to Eat Fried Worms," a film based on Thomas Rockwell's 1973 book of the same name.
