Holly Madison, 46, recently shared details about her cosmetic procedures, including a lower facelift, which she underwent after having children. Madison discussed the facelift and other surgeries on the Derm Approved podcast.

She explained that the decision to have a facelift was influenced by her experiences after welcoming her children, Rainbow, 13, and Forest, nine, with ex-husband Pasquale Rotella. "I just felt like after having kids, you know, I gained a lot of weight with my second pregnancy. And there was just nothing I could do," Madison stated, referencing her jawline.

Madison opted for a "lower facelift on my jawline" and "a little lipo" under her chin. She emphasized a desire to keep the procedure subtle, fearing an exaggerated result. "I'd rather have it not go far enough and then I can go back and maybe do more rather than have it go too far and I'm looking like the Joker," she said.

Madison took two weeks to recover from the facelift, with a nurse attending to her at a Los Angeles hotel. She acknowledged that scars are visible in front of her ears but stated it does not bother her, as she is open about the procedure. She also undergoes regular Botox, IPL laser, and CO2 laser treatments for the area under her eyes.

Reflecting on past procedures, Madison mentioned that breast implants can be "a tricky thing" due to potential complications such as the body rejecting implants, breast implant illness, or botched surgeries. She also described a hand rejuvenation procedure as a "waste of money."

Madison, who moved into the Playboy Mansion at age 21 and left seven years later, recalled that cosmetic surgery was common among other Playmates during her time there. "Hugh would pay if someone wanted it, so there was almost always someone in the house in recovery," she stated in an interview with Allure.

In 2024, on her Girls Next Level podcast, Madison revealed she had undergone cellulite removal surgery approximately five years prior. She noted that the results lasted about three years before cellulite began to reappear, leading her to consider the procedure again. She described the cellulite removal as a quick and painless process, but also acknowledged that "cellulite is totally normal, every woman has it."

Madison has also spoken about experiencing body dysmorphia, attributing it in part to her time at the Playboy Mansion. She told People in 2023 that living in the mansion made her question her appearance and feel inadequate. "Hugh had a way of making me feel like I wasn't pretty enough, and I would look around to everybody else and constantly be wondering, what's so different about them and why are they so much better?" she said. This led to a constant internal questioning of "what can I do? What can I change? How can I look better?"

Madison, who has previously had a nose job and breast augmentation, also discussed the impact these past procedures had on her confidence. She expressed concern that she could have ended up looking like a "plastic surgery nightmare" due to her past pursuit of perceived flaws.
