The adaptation opens with Cathy Ames, portrayed by Pugh, arriving injured on the porch of brothers Adam and Charles Trask, played by Christopher Abbott and Mike Faist, respectively. The initial episodes, titled "Brothers," trace the upbringing of Adam and Charles under their Civil War veteran father, Cyrus. While Adam is initially perceived as the favored son, tensions arise between the siblings, culminating in a violent confrontation before Adam leaves to join the army.

While Cathy Ames has historically been depicted as a sociopathic manipulator and "literal psychic monster," "East of Eden" reframes her character. Later episodes explore her past, suggesting her cunning and cruelty stem from experiences of sexual abuse. A sequence drawing parallels to Lewis Carroll's "Alice" books illustrates how childhood molestation shaped her worldview, leading her to believe that sexual violence is inevitable and that one should derive benefit from it.

Despite a potentially slow start, the series is lauded for its rich performances, poetic writing, and lush cinematography. Mike Faist, in particular, is noted for his blend of emotional vulnerability and technical control.

The series touches upon themes of sex and trauma, including scenes depicting the Trask brothers' encounters with a sex worker. The narrative culminates in Adam tending to Cathy's wounds, while her internal monologue reveals a deep-seated desire for invisibility as a defense mechanism against harm.
